What to Bring to Your First Visit

Please arrive about 15 minutes early for your first appointment so there’s time to complete your paperwork without feeling rushed. Our reception team is glad to help you fill anything in — just say the word.

Please Bring

Photo ID — a driver’s licence or other identification. Your dental insurance card, if you have coverage, plus your Medicare Advantage card if your plan includes dental benefits. A list of your current medications, including dosages, and any supplements — this genuinely matters, as many medications affect your mouth and some influence how we plan treatment. Your medical history, including any conditions, allergies, recent surgeries and your physician’s name.

If You’re Transferring From Another Dentist

Recent X-rays can often be requested from your previous dentist so you don’t need to repeat them. Give us their name and phone number when you book and we’ll take care of the request.

What the Paperwork Covers

A health history questionnaire, your contact and insurance details, consent for examination and treatment, and our privacy notice. Nothing unexpected — and nothing you’ll be asked to sign without an explanation if you’d like one.
